The dormitory conducts a self-health check survey to ensure the safety of dorm residents against COVID-19.
Submission of ‘Health Survey’ is mandatory for all dorm residents.
Please check your body temperature at the body temperature measurement machine,
and submit the survey until midnight(~23:59:59).

1. Wearing a mask in the dorm
2. Use hand sanitizer often
3. Avoid crowded places and refrain from unnecessary meetings and going out
4. Check the body temperature when going in and out
(In case of leased buildings, fill out the temperature record sheet)
